Determine the critical event that caused a failure and recommend a solution

Rules
1. Prior events, facts, and solutions must be relevant to the critical event or failure
2. Facts, events, and solutions should have data or other citation
3. At least 3 items or events should be included
4. Correlations must be causal (event A occurred because of action A)
5. Solutions must be related to the critical event
6. Facts, events, and solutions must be concise
7. Unresolved disagreements must be noted
